Requirements: Completed application to be submitted at least 15 days before event. Certificate of liability insurance naming the Village of East Troy as an additional insured in amounts of not less than one million (1,000,000.00) dollars for one (1) person and one million (1,000,000.00) dollars for any one (1) accident.
